This 51-year-old woman from Virginia Beach had gel implants placed in the 1980s.  She has been very happy with the results until the past year when she noticed the left breast appeared smaller.  It also became somewhat more firm.  An MRI scan revealed both implants were ruptured.  Because there was some hardening of the left breast and the possibility of capsule contracture, we decided to place new implants in a fresh pocket called a neo-subpectoral pocket.  I would still be placing her implants under the muscle but above the old capsule that went around her implant. Through an incision beneath the breasts, the old implants were removed along with as much gel as could be removed. The pocket that contained the old implants was closed off and new implants were placed above the old pocket but still under the muscle in a brand new pocket.  Her old implant size was 320 cc and we replaced them with 550 cc gel implants.  Although high profile, they give her the right dimensions for her breast width with results that are very soft.   This type of revision breast surgery requires very detailed planning and discussion with an experienced breast surgeon.
